Caring for you in the Home

Where it is your wish to live and die in your own home, our specialist team will do their best to honour that wish.

Many of our patients choose to remain home and to spend their last days at home wherever possible. Our specialist team can come to your home or provide support over the phone to ensure you have the care you need.

What to Expect from Hospice at Home

When we receive a referral, someone from our experienced team will make contact with you to arrange a time to come to see you, to do an Initial Assessment to determine how we can best help.

You may need help with managing your pain or other symptoms like mobility, shortness of breath or nutrition. Or, it may be that you feel anxious or worried about something, or want to talk to someone about your feelings or spiritual needs. Whatever your needs, we will put a team together to make sure you get the right care and support.

Our team always works closely with other providers involved in your care like your GP, consultant, pharmacy or other providers. Some of our patients can visit their GPs free of charge. Find out more by clicking here.

Some of the ways in which Hospice can support you in the home:
  • Supporting you to continue living independently at home
  • Providing 24/7 advice and support to help manage your symptoms or other needs such as mobility and nutrition
  • Helping you to navigate the various health and social services you may need, like your GP, District Nurses, your Pharmacy, Rōngoā or Counselling services
  • Providing emotional and/or spiritual support to you
  • Helping you with practical things such as funeral arrangements, wills and benefits
  • Helping you to fulfill any hopes and dreams that you may have and helping your to put an Advanced Care Plan in place.

Supporting you 24 hours a Day

You are never alone

Our experienced specialist team consist of Registered Nurses, Nurse Practitioners, Doctors, Healthcare Assistants, Social Workers, Chaplains and Whānau Navigators.

We are available to visit you in your home from 7.30am until 10.00pm every day, 7 days a week. After that, we have a Registered Nurse and a Medical Doctor on-call every night of the week to provide advice to you or your whānau on the phone. We also work in close partnership with St John’s ambulance, so in the event, that you have an unforeseen emergency overnight, we can liaise with the St John’s ambulance crew to put the right care in place for you.
