Partnerships Coordinator: Permanent Position – Full time

Here at Hospice Whanganui, we believe in supporting our patients to live the best life possible with the time they have left.  Through the provision of excellent palliative care, we support our patients and their whānau in achieving their goals and wishes as they journey through a life-limiting illness. 
We also believe in the value of partnerships and working together with other providers of services in our community to deliver the best possible care to patients and their whānau.  Hospice Whanganui provides specialist palliative care to patients and their whānau, but there is a wide range of other providers in our community who care for palliative patients and with whom we need to work in close partnership with.
We have an exciting new opportunity available for someone to join our amazing team as Partnerships Coordinator.  This person will lead the identification and development of professional partnerships between Hospice Whanganui and other providers such as GP teams, Māori health providers, District Nursing services and other District Health Board teams, Aged Residential Care Facilities, St John’s Ambulance, Home-based care agencies and other community services. The purpose of these partnerships would be to create collaborative working relationships that will benefit and serve our patients and their whānau.
This is a varied and dynamic role that will work with the inter-disciplinary team at Hospice Whanganui and link them up with other providers to ensure patients have the right support available to them when they need it.  You will also work closely with our Education Manager to ensure appropriate training and other resources are available to our partners to equip them to care for people with a life-limiting illness.
We believe that all people, regardless of who they are or where they live in our region should be able to access excellent palliative care and support.  For this reason, most of our work is done in the community, in the homes of our patients and their whānau and in the places that they choose to be.  We also have a small, five-bed in-patient unit where some of our patients may be admitted for short-term stays from time to time if they so choose.

The remuneration for this role is in the range of $78,000 – $81,000 annually.
