Hospice has a team of nurses who visit patients who are in their own homes or residential care. At any one time the community nursing team may be caring for 80-100 patients, most of whom are in their own homes.
The Community team will visit patients and assist with:
- wound dressings
- on-the-spot support and advice
- arranging outside agency assistance
- patients who are in hospital or residential care
Visits are as frequent as patient need dictates. The team will visit daily, twice or three times a week, fortnightly or monthly, with telephone contact in between times. Patients and family members can call for nursing advice at any time of the day or night if they have concerns.
Patients who are in need of end of life care may have frequent community nurse visits if they are needed.
We have a support group for family member/whanau who are caring for loved ones at home. With the Carer Support Group there is opportunity for family members/carers to receive support, information and resources. If you would like information about this please talk with your nurse.
